At Elevate Therapy Services, we recognize and value that everyone brings varying abilities to life’s daily challenges and opportunities. We work with our clients to ensure that the highest levels of communication, comprehension, and nourishment are achievable for each individual.

Elevate Therapy Services serves clients from birth through adulthood with speech, language and feeding intervention and therapy. We specialize in articulation, language, cognition, hearing, swallowing, social communication, and pediatric feeding disorder.

Our Process

Assessment

We come together in a safe and familiar environment to assess and understand each individual’s challenges and opportunities for growth.

  • Feeding assessment done at meal times.

    Breastfeeding and bottle feeding assessment done in the comfort of your own home.

    Collaboration with other providers and professionals to access full records and current course of treatment.

    Administration of standardized tests in some cases, including: PlS-5, GFTA-3, CELF preschool 2, REEL-4, DAYC-2, PTBI.

    Standardized tests are not always needed if client is self-pay provided the assessment environment is adequate.

Intervention

A personalized plan of action is created and put into motion.

  • An individualized treatment plan is created with family-centered goals that is ongoing and fluid to adapt to changing needs.

    Intervention strategies can include therapeutic exercises for improving oral motor and swallowing function, play-based intervention, and treatment plans that include physical therapy.

    The intervention plan is enacted in collaboration with the client’s family and team of providers.

Roadmap

A collaborative plan of care is built to address your individual needs and help guide you to successful outcomes.

  • Through collaboration with all involved care providers and the family, we outline steps to ensure our intervention tactics provide continued growth and achieve the highest level of function for each client.

    A timeline is established to guide next steps.

    Referrals are made when needed to address all points of care.

Continuing Care

After we build your roadmap to success, we work together to make sure your family has solid footing and the confidence to follow through. I check in to ensure the plan is working, and learn what I can do to continue to match the individual’s evolving needs.

  • After establishing a treatment timeline, care is ongoing in the form of:

    • Parent and caregiver education

    • Carryover stategies for home

    • Ongoing assessment as needed or indicated

    • Access to library of resource materials

    • Progress notes and reports

The ultimate goal of our relationship is to achieve outcomes that lead to patient discharge and a handoff of the care plan.

Our clients continue to have access to our resources as they navigate elevated functionality and independence.

Rates & Billing

We are currently accepting Medicaid and self-pay clients.

Payment is accepted at the times of service for self-pay clients.

Private payments could be applied to your out-of-network deductible and can be paid using your HSA or FSA.

Superbills are provided upon request for submission to insurance for reimbursement where applicable.
